Creating Fillable PDF Forms
Open Foxit PDF Editor and load the PDF you want to turn into a form.Click on Form in the toolbar.Choose the form field type you want to add:

Text Field: Click Text Field and then click where you want to place it.

Checkbox: Select Checkbox and place it accordingly.

Dropdown: Choose Dropdown and define the list items.
Customize the properties of each form field by right-clicking on it and selecting Properties.Once the form fields are added, save the document by clicking File > Save As.
